The Home Health Nurse will coordinate your medical needs through collaboration with your physician and health care team.
A nurse is on call 24 hours a day, seven days a week to facilitate your care for after hour needs as well as on weekends and holidays.
The Physical Therapist will provide services to improve muscular imbalances and other physical requirements needed for movement.
The therapists strive to assist our patients to restore physical functioning to help them feel secure, independent and self confident.
The Occupational Therapist will provide services to assist our patients in restoring their ability to perform the activities of daily living that are necessary for health promotion with or without assistive devices.
You have the right to select the home health agency of your choice. Patients can receive home health services through Medicare or most private insurances, when the following conditions are met:
- A considerable effort or assistance is necessary to leave the home.
- Home health services are prescribed by the patient's physician.
- Home health rehabilitation and/or nursing is part of the patient's treatment plan.
If you are a Medicare patient receiving home health services, Medicare covers 100% of those services.

The Speech Therapist will strive to address our patient's speech capacity, ability to swallow, communication skills and assist in the improvement in cognitive functioning.
Medical Social Worker
The Medical Social Worker will strive to assess the social, emotional and financial factors impacting the care of our patients. They will assist in providing solutions through the use of additional services and community resources.
Home Health Aide
The Home Health Aide will provide services to assist our patients in performing tasks necessary to maintain personal hygiene and a clean environment.
